Children's Case Management

Our Children's Case Management is designed to meet the needs of children, birth through age five, who have a developmental delay or children who are at risk of delay, or children, birth through age twenty, who have a specific diagnosis of Mental Retardation, Pervasive Development Disorder or Autism. Broadreach Children's Case Management is FREE to eligible children. Referrals to this program may be made by a parent, physician, school or another person with the parent's permission.

The Children's Case Management Program uses a collaborative team effort to enhance the capacity of enrolled children and families to achieve their identified goals. These coordinated services provide families with supportive links within the child’s home, school and community.

We work as a team with families to:

  • assess strengths and needs of the child and family;
  • identify needed resources and services;
  • support parents to enhance self-advocacy goals;
  • evaluate current services for effectiveness;
  • ensure children and families have access to identified services and resources,
    such as:
    • developmental services
    • respite care services
    • recreation funding and resources
    • financial assistance resources (heating, food, child care, legal, housing, transportation and other costs)
    • adaptive equipment for children with disabilities
    • parenting, child development education
    • community activities
    • local, state and national organizations
    • support groups
    • in-home supports
  • work with children's preschool program for kindergarten transition;
  • support the child and parents through an ongoing connection with the school;
  • assist in coordinating services such as preschool, child care, medical and dental care, specialists, counseling and legal help.

Our Case Managers meet with families in their homes, our offices, or at other community settings work with other providers as part of the Child and Family Team; are available when help is needed and have flexible hours to accommodate each family’s needs; and
provide services as long as a child is eligible, and the family chooses them.

Broadreach has been authorized by the Maine Department of Child Behavioral & Health Services to deliver Case Management Services. These services are paid for by Maine Care but funding may also be available, as resources permit, to children who aren’t eligible for Maine Care funding.
